Updated COVID-19 Office Policy
All patients wishing to see us in-person must provide either proof of full vaccination for COVID-19 or a negative COVID-19 PCR test taken within 72 hours. Patients unable to provide either of these two items are asked to take their appointments via secure HIPAA-compliant teleconference/video chat. We will consider exceptions on an individual basis for patients who wish to see us in-person but cannot meet either of these two requirements for a religious or medical-related reason.
All patients, regardless of vaccination status, must wear a protective mask while in the office. Masks should be worn properly covering the nose and mouth. It should be a surgical mask, cloth mask with 3 layers, N-95 or KN-95. Single layer cloth masks are not acceptable.
Only patients with appointments are allowed inside the office at this time.
